Join us for Vacation Bible School June 9th-13th from 6pm- 8pm, grades Pre-K through 6th. Please fill out the below form and click the Submit button.

If between grades, list the grade the child will be in at the start of the next school year.
List any allergies that staff needs to be aware of.
